Abstract

목적: 본 연구는 전문간호사가 수행하는 업무와 전문간호사와 함께 일하는 의료진이 전문간호사 업무에 대한 인식(난이도, 중요도)과 법제화 시 업무 위임 의향을 파악하기 위함이다.
방법: 본 상급종합병원에 일하는 의료진 277명(전문간호사 52명, 간호사 88명, 의사 중 교수직 51명, 전임의 44명, 전공의 42명)을 대상으로 하였다. 연구도구는 6개 업무 영역(건강문제 확인 및 감별 관련 6문항, 진단적 검사의 처방 및 시행 관련 3문항, 질병의 치료 및 악화 방지를 위한 처치 관련 18문항, 의약품의 처방 관련 4문항, 의뢰 및 협진 관련 3문항, 교육 및 상담 관련 6문항)의 총 40문항으로 이에 대한 전문간호사의 업무 수행 빈도, 업무 난이도, 업무 중요도, 법제화 시 수행 또는 위임 의향 여부와 적임자를 조사하였다.
결과: 전문간호사의 업무 수행 빈도는 건강문제 확인 및 감별 영역에서 가장 높았으며, 질병의 치료 및 악화 방지를 위한 처치 영역이 가장 낮았다. 모든 직군에서 업무 난이도가 업무 중요도에 비해 낮았다. 법제화 시 위임 의향은 건강문제 확인 및 감별과 교육/상담 영역에서 높았고, 진단적 검사의 처방 및 시행 영역과 질병의 치료 및 악화 방지를 위한 처치 영역에서 낮았으나, 전문간호사와 함께 일한 기간이 가장 길었던 교수직군에서 전문간호사에게 업무 위임 의향이 가장 높았다. 위임 의사가 없는 문항에 대한 업무 수행 적임자로는 의사군을 주로 선택하였다.
결론: 본 연구를 통해 의사군이 전문간호사에게 법제화 시 위임 의향이 있는 업무들에 대해 규명할 수 있었다. 이를 토대로 전문간호사의 업무 법제화를 위한 협의 시에 임상상황을 반영하여 효율적인 합의를 이끌어 낼 수 있기를 바란다.

Purpose: To identify the clinical role of Advanced Practice Nurses (APN), and evaluate how other medical personnel perceive their work(difficulty, importance).
Methods: A questionnaire survey was performed with 277 health care providers (APN 52, nurses 88, and medical doctors 137 [professors 51, fellows 44, & residents 42]) in a single, tertiary hospital. The questionnaire was categorized into 6 domains (total 40 tasks): 6 tasks on identifying health issues (A); 3 tasks on prescribing and conducting diagnostic tests (D); 18 tasks on disease treatment (T); 4 tasks regarding prescribing medicine (M); 3 tasks regarding medical collaboration (C); 6 tasks regarding patient education (E). The survey measured the frequency, difficulty, and importance of APN’s clinical tasks, and evaluated the willingness of authorizing clinical tasks to APN.
Results: The most frequent tasks for APN were A domain, lowest were T domain. The scores for perceived job difficulty were lower than those for job importance in all groups. The proportion of willingness to legally delegate clinical practices to APN was higher in A and E domains, but lower in D and T domains. However, professors, who spent the most time with APN, showed a higher willingness to legally delegate clinical practice. The participants favored medical doctors as substitutes for tasks which were not legislated for delegated job performance.
Conclusion: In this study identified clinical roles that medical doctors considered possible for legal delegation to APN were identified. The results can be used as evidence for the legalization of the practice of APN.
